Our Services

We work with patients in their homes across Bristol and surrounding areas, providing Occupational Therapy assessments and interventions using a person-centred approach. This includes practical advice, rehabilitation, equipment, adaptations, falls prevention, cognitive assessments, support and guidance for carers/ families, and more. We help people to overcome difficulties they are facing with daily tasks, such as accessing the shower safely or reduced confidence in getting out and about in the community. We also work preventatively with those who may not be experiencing specific issues now but would like to be prepared and set-up for the future. This is often especially helpful for those with Dementia and their families.

What We Do

We carry out a detailed assessment of the person’s needs and abilities, then in collaboration with the patient we create a tailored intervention plan. For example, a comprehensive falls assessment and follow-up support with organising grab rails and equipment to reduce risks identified.

Holistic home environment assessments

Rehabilitation at home; regaining independence in daily living skills and mobility

Equipment: from small aids to specialist equipment

Home adaptations such as grab rails, ramps, etc to help increase safety and accessibility

Functional assessments including showering, personal care, cooking skills

Transfer practice and advice, such as difficulty getting in/out of bed or on/off a chair

Falls assessments (including blood pressure review)

Mobility aids advice

Manual handling assessments and training, for carers or families

Seating assessments and postural management

Assisting people to return home following respite or a hospital stay

How We Work

Home Assessment

Once we have booked in a date and time that is convenient for you, we visit you at home to assess your needs and abilities within your own environment. Through assessments, discussion, and observations, we will help you to identify what specific challenges you are facing and create an intervention plan to address these issues. You are welcome to have a family member or close friend present during the assessment. This initial assessment takes between 1 to 1.5 hours depending on the complexity of your needs, which will have been triaged with you on the phone prior to the visit.

Tailored Goal Plan

Following this, we discuss recommendations and if needed we will set a goal plan with you. Your goal may be around increasing your confidence in going out for example, and so we would break this down into smaller steps and review your progress regularly, adjusting plans as needed. We can book in follow-up visit(s) following the initial assessment, or as we go.

Ongoing Support

Sometimes the next visit depends on equipment being delivered or an adaptation being fitted, therefore we would communicate with you by phone or email (depending on your preference) and review at a later date. If there is something that you or a family member would like to regain independence with or needs advice on, then please contact us. Patients can refer themselves directly to our services or families can contact us on their behalf.
